As a social prescriber, Jaiydeen connects patients with non-medical support and community based resorces to improve their overall well-being. In collaboration with healthcare professionals she enables patients to take control of their own health by indentifying their needs and introducing tailored interventions, known as the "what matters to me " approach. She tackles four main areas of patient concern: Financial support, Social Networks, Employment, and Housing issues. With a background in the social sciences and her previous role as a Patient Care Navigator she has a knack for connecting with others effectively, which in turn creates a safe environment for patients to work alongside her to improve their own quality of life.
